INQUIRY FINDS CLINIC THAT TREATED JOAN RIVERS MADE NUMEROUS MISTAKES: A federal report on the death of JOAN RIVERS concludes that it's possible she died of too much propofol.

In a report released yesterday, investigators said there's no evidence Rivers was weighed before undergoing procedures on her throat. Getting an accurate weight is key to knowing how much propofol should be administered. Records at the clinic show Joan received 300 milligrams of the drug, but an anesthesiologist claims she actually received less than half of that.

Investigators also found the medical team failed to notice Joan's blood-oxygen level had dropped below normal for a full 15 minutes before they started trying to resuscitate her.


The report also includes details we had already heard -- that Joan's personal physician performed a procedure on Joan even though she wasn't licensed to do so at the clinic, and that Joan had only consented to one procedure, but was given two.

MELISSA RIVERS issued a statement saying she's "outraged by the misconduct and mismanagement" at the clinic.

CRANBERRIES LEAD SINGER CAUSES DISTURBANCE: CRANBERRIES singer DOLORES O'RIORDAN was arrested yesterday, after assaulting a stewardess on an international flight. O' Riordan was flying from New York to her home country of Ireland when for some as-yet unknown reason, she freaked out on the flight crew.

Dolores assaulted a female flight attendant, who reportedly suffered a broken foot, and then when they landed in Ireland, she head-butted the cop who tried to arrest her. She has yet to comment on the incident, so we don't know her side of the story.

Police are still investigating, and Dolores has not been charged with any crime yet.
